If the IT system does not find you insured, or you need written proof for a hospital or a job, you can ask for this certificate.

If you enter your town or city, we can look for requirements specific to your area.

  • ID card copy
InstitutionThe county Health Insurance House (CAS).
How to submitOnline (through the CNAS portal) or at the counter.
NoteFor employees, the certificate can often be downloaded straight from the CNAS SPV.

Privacy: your data is not sent anywhere. What you type stays in your browser and is kept only while you fill in the form; it is deleted when you generate the request or choose “No, delete”.
